If you or your group has developed a survey and would like PRiSM to distribute it on your behalf, fill out the form below and email it to info@prismsports.org. Please allow for 6-12 weeks from the time of submission to the time of distribution. Once the submission form and survey is reviewed by the Research Committee, we will contact you with more information.
